PART I – CONTACT INFORMATION (this information will not be made public on the AYP website)
PART II – TRAINING INFORMATION Please list the following information for all yoga schools where you have received training. (Note: A “yoga school” can be a yoga studio, university, community college, ashram or any other institution that offers Yoga Teacher Training. Please attach any relevant certificates, transcripts or letters of recommendation with your application. If none are available, please explain the reason why.)

PART III – PERSONAL PRACTICE & TEACHING QUALIFICATIONS Please tell us a bit more about your personal practice and teaching experience.
PART IV – DOCUMENTATION OF TRAINING In a few sentences, please give a summary of the coursework you have completed for each of the following curriculum areas, as well as any supporting documentation. (Note: Graduates of Teacher Training programs accredited through The AYP are not required to supply additional training information in order to obtain membership. If you completed a Teacher Training program that is not accredited through The AYP, in addition to your certificate of completion, you will need to complete this section of the application, describing the nature of any training you have received in each of the curriculum areas, as well as any specialty areas. Documentation includes certificates of completion from any trainings you have attended, official course outlines or syllabi to demonstrate curriculum content, and/or letters of reference from your teachers who can verify your experience. Yoga History & Philosophy: Please describe your study of any influential texts, teachers and/or relevant historical circumstances that contributed to yoga’s evolution from its earliest known origins to its modern form.
Yoga Techniques: Please describe your study of yogic techniques such as asana, pranayama, application of bandhas, mantra, mudras and/or other techniques. Particular emphasis should be placed on describing not only your experience with these techniques, but also your understanding of the science behind them.
Teaching Methodology: Please describe the extent of your study of various teaching methodologies such as cueing, observation, demonstration, hands on adjustments, different teaching and learning styles, holding space and any other methodologies you feel are relevant.
Anatomy & Physiology: Please describe the extent of your study of the anatomy and functional physiology of the human body, including both the musculoskeletal system, as well as the nervous system.
Subtle Body: Please describe your study and understanding of the yogic concept of the subtle body, including the nadis, chakras, koshas and gunas (study of Ayurveda not included).
Yoga Ethics & Lifestyle: Please describe your study and understanding of the ethical principles of yoga, as well as the role of yoga teachers in communities and society at large.
Teaching Experience: Please describe your teaching experience to date.
The Business of Yoga: Please discuss your experience in the business of yoga. Include any thoughts or perspectives you may have about improvements that could be made to the way the business of yoga operates, as well as any dilemmas you may have faced.

Payment & Membership Number Once your application has been completed and accepted, you will be given a membership number that you can use for discounts on various products and services. The AYP cannot issue your membership number until we have received your membership dues and any appropriate administrative fees. Declaration: I have read and understood the requirements for joining The Association of Yoga Professionals and declare that the information I have provided and attached to this application is true, and that my training and experience meets the requirements for membership. I understand that acceptance of my application and renewal of my membership each year is at the sole discretion of The AYP and its Board of Directors. I also understand that if any aspect of this application is found to be false, incomplete or otherwise misleading, membership may be refused or revoked at the discretion of The AYP and its Board.
